Depends on the type. Some have a removeable cap or eraser and you place the leads down in the hole. Others seperate in the middle and have a chrome tube with a screw on cap you remove and place the leads in. Some you stick the lead down the tip of the pencil and push or rotate to retract the lead into the body of the pencil. Some you hold a button which may be the clip or on the end like an eraser that you hold down to release the old lead and then you put the new lead in the tip and release after pushing the lead all the way in.

If you buy the cheap type I do, you can remove the eraser off the top and insert the lead into the hole it reveals.
