What's the question?

If the child is not being cared for, then call social services.

I know they are your family and you don't want them taken but if it's the safest solution it has to be done. What if, in their neglect, one of the children eat poison or put their finger in a socket, you know the millions of possible situations that a child could get seriously hurt or lord forbid worse. These children are your blood too. Put yourself outside the box for a moment, and you where an observer or maybe even social worker what would you do? Take the children away? If the answer is yes then make the call and save these children, until their parents take their jobs and roles seriously. Do they even realise that these are souls they are chancing with? Not some cute like play house fantasy, but real life long responsibility's. Please, I hope you do the right thing. If something bad happened, you would be partially to blame as well for knowing the situation and not helping.

then talk to them don't take drastic measures of taking custody. Talk to them simply about you helping a little bit. The mother not taking meds, alot of younger people don't liek the take meds it bothers them as they associate meds with older people. I know i do this all the time. I don't have bi polar no.. but i do have my own health problems.
If i were you i'd sit down adn admit some mistakes to them first when you sit down to talk to them. Obviously, the dad is trying and that is the main concern. There is no manual to raise a child. Children are dirty all the time. My youngest son can get a bath and 5 minutes later is dirty from head to toe. Kids are kids adn like it or not kids get dirty. Let the kid have fun and play. If their house is dirty there are organizations that will help them clean or learn to clean the house properly and how to maintain it. I would first try to help as much as you can.
Please remember parents are very judgemental of their children. they want the best for their children and some can't admit they even made mistakes. a lot of them think they are the best parents.

Then do the right thing! Be there for them. Offer in a nice way to take the extra load off them and take home some laundry to do. Offer to watch the baby for them to get rest. If you know they are cleaning the house that day pop in to help. Take an honest interest, not their child. bring by a bag of food or dinner.

If the child is not being taken care of, for example: is very dirty, than you should call Child Protective Services and try to take over custody. But I would do my best to work with your son and daughter-in-law on helping them out first.

you can always make an anonymous call to child protective services. they will investigate and make sure the child is given proper care: i understand not wanting to get too involved. but you have to think of the child and what course of action will benefit his future!!!

if you feel the childs well-being is being compromised then you should interfer because if you don't, that makes you worse than them because you can see what is happening and are afraid to help!!!
