Clarification:

4 years of med school gets you an MD
variable years of residency gets you a license and board certified to practice. The first year of residency is called your intern year. (This goes to the day when new doctors did 1 year of general medical work before doing their specialty training. Now, it just refers to a first year resident.)
You pay to be in med school
Interns (1st year residents) typically get around $40,000-$45,000 for the first year, and the salary goes up by maybe 1-2 grand a year.

Compulsory rotating interns are paid from $500 to $1000 a week., get free meals during duty time and permitted free parking. Intern is one who can not leave hospital premises.

internship is the 3rd and 4th year of medical school. You do not earn a penny, but instead you still have to pay the 30 grand in tuition a year.

You pay for 4 years of med school.
Then you get paid for the residency $38,000 - $45,000 / year roughly. It's different in different places. Some a little more.
