Yes, every doctor will be able to tell that you smoke during a physical. And no, the doctor will not tell your parents if you do not want him to. Just make sure that they are not in the room and that you are honest with the doctor.
PS Smoking really is horrible for you and will cause you loads of problems later in life - even at a pack every 2-3 weeks.

They don't test for nicotine in a urine test. Your doctor may be able to tell though, because smokers smell like smoke. Perfume doesn't help, it just draws attention to the fact especially in a sterile setting. The good news is that your doctor has to obey something called doctor-patient confidentiality. So he can't tell your parents. But you should quit now for your own good. After 15 years of smoking I watched my beloved mother-in-law die an awful death from throat cancer and I still can't quit, even though I really want to. Get out now while you still can!

Yes they will know. Do youself a favor now, while it's still early, quit! I could go into all the reason why, but just try this, take an ashtray and put your nose in it for 15 seconds. If that doesn't cure your desire to smoke nothing nor anyone telling you will make you quit. I smoked for 35 yrs, quit 7 yrs ago and thank goodness I don't smell like that anymore, I can breathe and my sense of smell has returned, not to mention all the other health benefits. just quit, just do it and do it now.

The only way they will be able to tell is by your breathing and doing a lung test which is having you breath into a small machine. There are no urine or blood tests that are going to show you smoke. You don't smoke that much anyway.

Possibly it depends on the tests the doctor performs.. and quite honestly shouldn't you fess up? I never lied to my parents - and they deserve to know that you're smoking (to be honest, I wish you weren't - I have lost a lot of friends and family to smoking, but it's your personal choice).

I don't think they are allowed to tell your parents your health issues. But that is just my thoughts. If I were you I would simply own up to it in the exam - your doctor needs all your medical information to treat you properly and ask the doctor to keep this information to their self. If they have any ethic they will do that for you - and they will respect you for respecting them and your honesty.

yes your doctor can tell that you smoke when they ask you to breathe in and out and it is harder for smokers to do that than non-smokers. second, they can see how much build up you have in your heart from all the chemicals in the cigarettes. so yeah, i dont think you can get away with it.

It only takes one blood test and your Doc knows more about you than you ever did.
Oh yes and if he or she doesn't smoke they will know as soon as you walk in.
