First - there is not State of ILL. Well, actually, judging by your spelling and wording of the question, perhaps you are in a state of ILL.
Second, why would the state want to obtain a medical card? Aren't you the one that needs to obtain a medical card?
If so, then here is where you need to start:

You may want to either lay off the Vicodin and/or liquor before you call.
