Filling out forms these days seem to imply that it is mandatory to have a First name and Last name. In my culture we usually have a single name and I have heard of people facing problems while applying passports, visas etc. I have also seen some forms where first and last name is mandatory and lack of which cannot be processed.

Is there any constitutional or legal bill (of any country) that says a citizen must have a first and and last name? Or is a citizen legally required to have a name at all?

You can go to court and ask a judge to change your name to what ever you want and if he is willing you can legally have a new name. Only one if you want. Even numbers if you want. I heard of someone changing their name to the decimal version of the infinitive pi. (3.14159265358979323846...........)

there doesnt appear to be a law requiring you to have a name. in your case, you may want to write your name in both the first name and last name spaces.

I've heard of someone who changed his name to a number.
And who can forget Prince changing his name to a symbol.

There's a way around it. Madonna's legal name in the US is Madonna.
