It depends on how far you are planning to pursue them.

Economics or medicine offers no major monetary payoff without advanced degrees beyond college.

Physician is clearly not for everyone. It requires 4 years of college, 4 years of medical school, and 2 years of residency. It takes significant amount of time, effort, and resource to become a medical doctor.

You can become a certified nurse after 4 years of college plus one year of accelerated master's program, but nurses do not earn significant amount of money when compared to physicians or accountant.

Business degrees like BBA and MBA require you to have had economics, but successes in those programs and later in business career do not hinge heavily on having excellent knowledge of economics.

Accounting, on the other hand, will help you obtain jobs immediately after college. Five years of schooling (4 years of college plus one year without masters) will qualify you for CPA exam.

Should you choose to purse advanced degree in accounting, a Ph.D in accounting pays lot more than a Ph.D in economics and is more employable than Ph.D in medicine (Ph.D in medicine is a research job).

Compared to other choices, accounting has a limited downside and a very high upside.

It depends.

What do you like doing? You won't be happy in medicine if you are too empathic because you will see too much suffering.

Will you be the top person in the world in your field or will you be closer to average? The world's top economists have lucrative and exciting careers but most people who majored in economics are not in such influential careers. I think medicine would be the most interesting if you expect to be more in the middle of the pack because you will always be presented with problems outside your specialization and there is a huge human element to the work as well.
