I haven't seen the commercial and I'm not sure that the show has actually started yet this fall.
However, if you're simply going by what you saw on a snippet of a commercial, then I wouldn't get your panties all in a twist. I've seen commercials for shows, and when I actually watched the show, I found that I'd been duped, and the ad had little to do with the show.
Wait and see. THEN make up your mind whether or not House has jumped the shark. I don't think it has.

I wouldn't make any presumptions about House until the ep actually airs. What they show in the commercial could be the kid dreaming, or a flashback or something... Nothing in House is ever quite what it seems - remember the ep with the teenage female model?? I STILL haven't gotten past that one!!

I don't think so, I'm sure there's an explanation for it. The writers are just exploring new ideas this season, trying to get away from the typical stuff they've been doing the past two years. I'm looking forward to anything they want to do, it's my favorite show!

The second I saw that alien commercial I knew it jumped the shark.

I never thought it was very good. Then again I am not a fan of television dramas as a rule. Not that sitcoms are any better either.
