It would not be too hard, i would recomend using a very wet mix or a topping compound of sorts so there will be no signs of segregation, concrete is also very britle and is prone to cracking/snapping, so steel will be a must.
I would advise you build a strong timber structure below.

We want to do the same but haven't yet. Tauton press makes a great book on the subject with lots of different ideas and step by step instructions.

I'm pretty sure you need a custom mold for that, and a very high density concrete.

you can purchase concrete countertops now just check with your local kitchen remodel store or an interior designer
