It depends on the jurisdiction. When asking legal questions it really helps to say where you are.
If there really is an emergency, pay the grand and get a lawyer to do it.
The types of emergency in child custody cases can have dire consequences (such as a child being removed from the jurisdiction, never to be seen again)

You don't need to prepare this document in a legal form. Just write a letter and be polite to the judge. The court will honor this letter. Once received, the court will order you to do additional things, like having it notarized, for your document to conform to the legal requirements. However, this ordinary letter can start the proceedings.

depends what state you are in for example Michigan has the friend of the court. all you need to do is go to the court house and they do the rest for you. You may want to see if they have a child suport recovery dept. they too will get things going. Oe just go to the court house the ladys there will help you. you have to file.make a court date send a notice . they will help its all done there just tell them you wnat to do it your self

You can go to the courthouse and they have all the forms you need also that you can fill out on you own.
