I would imagine it takes some effort.
We live in a society that worships celebrity. It's insane how much people care what celebrities are doing/wearing/eating/etc. It HAS to affect you to have people so involved with your life. I would imagine even the most balanced people start to get weird feelings of (false) importance. Celebrity and money can do far more in our culture than they should, and while some celebrities start off egotistical, i'm sure there are others that just start believing their own publicity.

1. Yes. Like anything you succeed at in life, you have to have an absolute self-confidence in your ability to make it happen.

2. You have to be able to let criticisms roll off your back.

3. You have to have nerve to stand up to the masses, even if they 'boo' you.

4. I would rather use 'tremendous self-confidence', rather than 'ego'. There are many people walking around with egos the size of the universe, but unable to do anything of remote significance.

yes, i know a couple personally and they don't have big egos at all. they actually use their celebrity to further great causes and remain humble in the process.

Yes. For example, Leonardo DiCaprio doesn't make assistants do his shopping for him, he actually goes to the store himself. Harrison Ford and Robert Redford come to mind as well. They live in the middle of nowhere, far from the throngs of fans. Look at Renee O'Connor, who played the sidekick on Xena. She quit acting to become a mom. She hasn't acted in years.

Possible, but difficult. If you have people telling you how awesome you are all the time, I'm sure you start to buy into it. It's human nature.
