contact your medical card company and let them know you lost your card and would like to recieve a new one in the mail. if you dont have the information or phone numbers needed to contact them, then go to the last hospital that you used your medical card at and explain to them your situation and they should give you a copy of your records that will have a phone number or address you can call or write to to inform your medical company of your situation to get a new card. good luck.

When you register with your GP you will automatically be sent an NHS card from the Local Health Authority. . If you have lost yours or cannot find it you can obtain a copy from your Local Health Authority. You can obtain the number from your GP practice, the telephone book or directory enquiry.

If they have extra details you will have to pay for the report.
