You could say that you got a better opportunity somewhere else or that you felt you had learned all you could from that job or that you are looking for a new experience or that you were at a transitional period in your life.

You should list your job history for the last 5 years (if Applicable).
If you list too many job your future employers might not think you can hold a job. When asked why you don't work there anymore you could say.... needed more hours.......left for higher pay rate.....family emergency,if you have had children between job, you could say maternity.

Put your job history for the last 3-4 years. In regards to your "reason for leaving" portion of the application I would be honest.

the best way to say you were fired is to put "laid off". if you left a job because you didnt get along with someone, especially the boss, put "personal reasons". they legally cannot ask you what that means. if you left for another job, put "offered a better paying position" or "offered a job closer to home" or whatever the reason is.

I always am honest, I have left jobs because I hated it or did not get on with the employer.
So I write,
An opportunity arose to enhance my own personal and professional development.
Its the truth...this is why we all move on...please do not ever state the facts of the matter, its only to get the interview, so once you have that interview and your reference is in their hand this is when you elaborate, but only if asked, a good elaboration is that you felt that your skills were not fully appreciated and that there were few opportunities for professional development or career progression.....
Its the truth...

If you quit, say you were offered a better job and you had philisophical differences with you're boss.
If you were fired/let go, say that you were laid off.
