Well, the strengths part is easy. Go with some combination of these: organized, hard worker, get along with all sorts of people, can work well alone or with minimal supervision, dedicated, prompt, dependable, learn quickly, etc.

It is better to select "weaknesses" that you have recognized and taken steps to address. For example, if one of your "weaknesses" is organization, you could say that you recently took a time management course and are putting it into practice, or you sat down with your last boss and worked on "to do" lists and are doing well, or other tangible steps so that you really don't have this "weekness" any more.
Please don't use the really tired "I'm a workaholic" or "I'm a perfectionist" lines - they are not really believeable.
For the perfectionist one, you could say "I have had a problem delegating work, since other people sometimes don't have such high standards as I do," and then described how you have mentored someone or successfully delegated recently to work on this.

Please use true answers from your own experience. Above all you must be honest in an interview.

I believe you should know the answer for both but I do suggest that during the interview you should not boost on your strength. You should work on it by showing that you are willing to learn more things even though you maybe strong in certain areas. In this way your prospective employes may think differently about you.

Well that all depends on the person but some good adivce someone once told me was when telling them my weakness to make sure and put a positive spin on it. Or just say this is my weakness but these are some ideas I have tried to improve upon it.

You should list your strengths as you see them and don't tell them that you are great at something you're not good at. It could come back to haunt you. You should also honestly list your weaknesses and let the employer know that you are always willing to learn for the betterment of yourself, your career and the company.

I always say "If I have any weaknesses it would be that I am too much of a perfectionist, and people like working with me too much sometimes."

My strenghts is that I strive for perfection.

My weakness is that I have to realize that I am not perfect and everyone does not think as I do so, I cannot expect for all to be perfect.
