What you would need to do is either call the police, or file a civil legal action to have the person removed.

If the person is already on and remaining on the property, the action is usually called "ejectment". If the person keeps coming onto the property, and it's your property, then you would get a retraining order.

In either case, your best option is to consult with a licensed attorney, who can help you submit the proper pleadings to the court.

A form? Why not just call the police and let them deal with the person in question?
