As others have commented, the most common is going to be lack of oxygen, often related to circulatory problems (such as congestive heart failure).
HOWEVER, if you're wanting something medical AND a little offbeat: be aware that use of certain metal compounds can cause a bluish or greyish coloration in the skin itself. Back in the 1930s, mercury compounds were used topically as skin whiteners and freckle removers---and some people developed significant skin discolorations as a result. I THINK that some folk nowadays using colloidial (spelling?) silver internally have had similar problems.
If you're wanting a REALLY blue tinge, just for giggles, go read _Eleven Blue Men_ by Berton Roueché. (You wouldn't believe what caused the men to be really, really, really blue....Medical detection stories, LONG before *Quincy*. If your library doesn't have a copy, Google the phrase--you might be able to find the story online. COOL reading!)
