The biggest source of miscommunication I can think of is bias media. If you listen to the things that the media feed you you will always be afraid of everything. Go out and live your life and pay no attention to the man behind the curtain.

Just someone talking ... to me thats one of the biggest sources, especially when someone walks into the middle of a conversation and hears something and gets upset because they think that the person(s) are talking about something else.

The universal line, "I'll call you." It basically means nothing. The person saying it could mean they'll call you tonight, tomorrow, next week, next month, next year or they really have no intention of calling you. This is the one that keeps making women say, "Why hasn't he called?" (Duh. Just because he said he'd call means nothing.)
