i did lot of practice but when exams *** really i feel nervous to c the paper and i lose my concentration.

When you have an exam, fibd the easiest question to answer and answer it. This will help you settle and build up your confidence. Then continue with the rest of the paper, continuing with the questions you are most confident about. The most difficult questions are left to last. By the time you get to these you have difficulty with you have built up your confidence and you should be able to make a good attempt at theese too.
A good idea is to pray, ask God to help you. The bible says "if any one lacks wisdom let him ask of God who gives abundantly and criticises not". Also ask Him for His peace and confidence.

you need to find away to calm down before your exams then. eat a healthy and balanced diet, get plenty of sleep and exercise and find a way to relax right before your exams. i find listening to ramones right before i go in always cheers me up and calms me down, but you'll figure out whats right for you. good luck.
