I think this it is the most shocking incident:
Heysel Stadium disaster
From Wikipedia, the free encyclopedia

The Heysel Stadium disaster occurred due to football hooliganism in which a retaining wall of the Heysel Stadium in Brussels collapsed on May 29, 1985 during a football match between Liverpool F.C. from England and Juventus FC from Italy. 39 people were killed, mostly Italian Juventus fans. The match was the final of the 1985 European Cup tournament.

About an hour before the scheduled kick-off time, Liverpool fans breached a fence separating the two groups of supporters and charged. Juventus fans were forced to retreat, putting pressure on a dilapidated wall, which collapsed on top of them. The game was played despite the disaster. Liverpool were later banned from all European competitions for six years and all other English clubs were banned for five years.

The disaster has been called "the darkest hour in the history of the UEFA competitions

Can't think of his name but a bloke just keeled over and died on the pitch in a match in France a couple of years ago. The teams had all been complaining because of playing in the mid-day heat as well.
Dave Bust breaking his leg in the Manchester derby was pretty bad as well.
And of course, Andreas Escobar being shot for scoring an own goal against Colombia for USA.

The Ben Thatcher 'tackle' should be up there and the Gereman keeper Schmakker's chest high challenge on a French player (can't remember his name)
