Sexual desire is a normal and human issue and masturbation or sexual act should be the logical consequence, right the normal need. I think the castration only gives the definitive solution your brother is looking for.....

He wants surgical or hormonal castration. That is the removal of his testicles or taking hormones everyday. He should see a urologist for these options. I am certain any responsible urologist will send him for a psychological examination first.
