Try a nail. I used that for a pinhole camera I made out of a tin, and it worked well. Don't completely drive the nail through, just tap it until you get the size you want.

How small is small?
A needle from a syringe could make a hole, but it's not likely since that would likely bend the needle before the needle could pierce the cap unless the movement is done exactly right.
A nail or brad could make a hole.
A dart, either thrown or fired from a pellet pistol (different kinds of darts, of course), could make such a hole.
The tip of a knife blade could make a hole.
A punch could make a hole.
A power drill with a high speed bit could make a hole.

Hot needle? That way you could melt a small hole and not damage the needle too much.
