Then you'd reply Planet Earth. Then the other person would say: 'Yeah, but which solar system?'

If you think human nature, attitudes and needs will ever change... forget it.
All that changes is the fashion of the day, the technology of the day progresses, but eventually it rusts, corrodes and crashes... because it's made by man and unlike the planet that we live on, which was made by god to self renew itself, everything we are and make and own, will wear out, fade and rot away eventually.
So when we traverse to other places, we take our habits with us, and what goes around, comes around... since man has but two arms and legs and so clothes are limited in design to return, and return again to what went before it.
And as we only have certain basic needs and live in certain basic ways, we will always make the same mistakes and use the same jargon and build for our comfort and protection.
Village, state, county, what's in a name? In the sky, on the ground, underground, we'll still be people with the same need to reinvent and reinvent... and think we got there first, but we didn't... its all been done before and will repeat itself for ever, until the end of time.
Only the creator has the power to make it change, not conceited man;-)

If by some chance this ever happens, these settlements will likely be considered colonies, much like the colonies in America from Europe. So if say a group of American's are sent to a colony on the moon, they will be colonies or territories of the United States. Same for China, Russia, England or France.
Maybe there would even be a space revolution where the colonists insist on governing themselves. Unlikely though since they will be dependent on Earth-based governments for resources.

Just take a look at the place names in the USA. Paris, Texas is just one of the interesting anomolies. New Hampshire, New England. And even stranger names. But maybe we would return to our Science Fiction roots, wherein most planets are named by the settlers - who have been less than impressed by their amenities, or overly confident of the outcome.

If you were in England and asked someone where they came from and they replied Birmingham you would accept it.
If you asked the same question in America and got the same reply you could quite rightly ask them what planet they were from.

We have already created so much mess on planet earth (in terms of wars) by creating geographical boundaries. I pray let extraterrestrial sites i.e. new planets be Human kingdoms....common for every homo sapiens...and not territorial differentiability into nations!!!!

i think it would be known as a colony and have a dramatic sounding name (especially if it was our first colony) like 'New Earth' or 'Hope' or something equally impressive, they would develop their own society and rules etc. they would have to make sure earth is still able to support them if need be - imagine the only food etc supply being cut off and being several million miles away! sounds like you are a person that would enjoy science fiction. keep your mind open and keep thinking creatively.
I think that if we went to another planet, we would have new names, not the same ones.
