A toner is used for many different purposes. It makes your pH balance even, takes off any traces of dirt, make up, or oil, and it closes your pores so no more dirt can enter. The correct order to do things is to cleanse, tone, mostuirize, then put whatever treatment you put on for your skin. You want to do those steps twice a day every day. You can replace your cleanser with a scrub three times a week. Also make sure you use a mask atleast twice a week.

Cleansing, toning (if you feel you need it, read on) and moisturizing. You can use a scrub after cleansing at night if you wear makeup, or I just use one in the morning. How often depends on your skin (every day is fine for some while others only need it once a week) and whether or not you are also using a chemical exfoliant such as salicylic acid.
You don't NEED a toner. Most toners out there are full of alcohol, which is very drying and irritating to your skin and should be avoided, along with anything containing witch hazel. The purpose of a toner is to remove last traces of makeup and/or as a chemical exfoliant. They claim to close your pores which is not true...toners with a lot of alcohol will slightly irritate and swell the skin around the pores, making them appear smaller. This doesn't have any benefit for your skin!
One toner I would recommend is Clinique's mild toner...it's not one of the 1-4 series (those have a lot of alcohol) but in a green bottle. It's gentle and contains salicylic acid which will help exfoliate your skin.

Toning should be done after washing and exfoliation, and before moisturizing or adding makeup. Toner restores the natural PH of the skin, and closes pores.
A good scrub should be used once per week. Scrubs are abrasive, and can cause irritation, dryness or aggravate acne and make your skin even oilier. If you notice any of these problems, switch to a gentler scrub, or use every other week.
