who is public information officer in Department of health and family welfare, tamilnadu ?

under Rights to information act, there is a public informaion officer in each department. I want to know the public information officer in dept of health and family welfare, Tamilnadu to get few information from him. what is the fee to be paid for him for enquiry, and in favour of? Is there any prescribed form or application?
