If you can send the dog with someone, that is cheaper and easier for the puppy to travel as luggage than as cargo. We did that last month (had friends bring a dog to AK from Maine) and it cost $180 (depends on weight and size of kennel. That was Continental, 60# lab in a fairly big kennel. They no longer require a health certificate (but ASK each airline). Another poster spoke highly of them. Alaska would be another choice, at least as far as Idaho Falls or Billings (on Horizon).
Small dogs can go in the cabin, but (obviously) would have to accompany a passanger.
In cabin, as luggage or as cargo, you need to make a reservation because they have limits on how many animals caan be on the plane. There are also temperature limits. Not forecast to be over 85F (I'm sure) or under 20F or 30F (?) at origin, destination or transfer points. So if a cold front moves in, they might not take them, even with a reservation.
If not, you can send the dog on an airline as cargo. They travel in a pressurized, heating part of the plane. (p.s. ALL of the plane is pressurized, but some cargo holds aren't heated). It runs $250-500 but call several and check.
Ideally, get the dog used to a shipping kennel beforehand (you have to use the official plastic ones, not a cardboard box). Give the puppy treats in it. Then take it to the park to play, etc. Make it a positive thing.
