It is tradition that dates back a 100 years. Before modern advances, such as anesthetic drugs, operations were a very brutal affair, the fatality rate was very high and most patient were fully conscious during the operation. Not surprisingly because of these reasons the general public were terrified of seeing a surgeon and practicing physicians regarded them as nothing more than a glorified butcher, if that. Surgeons learned their art on the job without having to attend medical school and so they weren't strictly speaking "medical doctors". I guess surgery in those days was a kind of alternative medicine.
A lot has changed and surgeons are now highly respected for their role in medicine but in Britain some of the age old traditions are still being practiced with timeless fashion, even if it means replacing the "D" for a "M"

maybe because that Dr holds a master degree, thats why they addressed him as MR.
