Sanctification of life ("Kiddush ha'chayim") is a very important value in Judaism, and it allows the Jewish believer to violate almost any religious law in the name of life. When a Jewish person is in a real danger ("Piku'ach nefesh") he must break any mitzah that may put his life in risk, except of three unbreakable rules that all people must implement, according to the belief :
*blood shedding: killing other people.
*forbidden sexual relations: incest.
*idol worshipping: paganism.

Yes.
Pigs are not kosher for eating. It is permissible to use them in other capacities, e.g. a pigskin football.
This was something a Grey's Anatomy episode last year got wrong. (i.e. a religious Jewish girl in the episode refused the transplant)

It will depend on his/her beliefs and the religious doctrines of the Jewish Culture he/she was raised in. Either way, it's his/her choice if they can bare the idea of having a part of a livestock animal inside his/her body.
