They are very successful, but they are not foolproof.
There are a lot of factors that go into a successful implant-supported prosthesis, ranging from proper treatment planning to proper surgical technique to proper design of the prosthetic tooth. An implant placed at an inappropriate angle will endure forces that are not axial in direction, and such an implant will be more likely to fail. Similarly, an implant with an excessively large prosthetic tooth to support is also more likely to fail. Also, adequate supporting bone is also a must. Lots of things dictate the success or failure of an implant.
Patient health is also a factor. Uncontrolled diabetics, patients with severe immune deficiencies, patients with insufficient bone density are poor candidates for implants. Patients with poor hygiene practrices, and...and...and...
You get the picture.
When done properly and on suitable patients, they have a very high success rate. Your doctor needs to evaluate your situation.

Dental implants usually have a success rate of about 95%. Which is really good odds. Smoking can decrease those odds by about 2%, which is still good. Excessive pressure, especially sideways, is one thing that can make an implant fail. In our office we have only had 2 in the last year fail, and that is out of about 75. The chances of an implant failing twice is remote. Implants are usually called "30 year" implants, because that is usually how long they are monitored. Of course, they can last longer than that, but most dentist retire by that point or the patients moves or dies, and obviously it is not monitored any longer. Implant placement is not anymore painful than an extraction, and usually even less. In our office we can place an implant in about an hour, and that includes anesthetic and post op.
We use Astra implants, and they are great.

First you have to go to an oral surgeon, they will put a metal post in your gum. Then you have to wait 6 months for it to heal over before they can put a fake tooth in. Its quite a procedure.
