Obviously, if you want to vary the sentence structure, you have to choose the sentence which has a different structure from the sentences on either side of the blank.
So you don't want to be using a sentence that starts with "Anna is..." (which excludes B and D), and you don't want to be using a sentence that is like "Anna does..." (which excludes C). To put it in a more grammatically precise way: B,C and D, just like the sentences on either side of the blank, all have "Anna" as the subject, while A is the only sentence in which "Anna" is the object. Plus, A is the only sentence which is a question. So you want to choose A if you want to vary the sentence structure!!!

I am not sure what they are asking you to do, but D is the only one that you could say INSTEAD of the sentence before the blank and the one after it.
A is the only one that would make all three sentences sound like an acceptable paragragh, because it uses a pronoun.
