Polio virus enters through the oral route and infects the intestinal epithelium. After replication here, it causes a primary viraemia where it infects many different cell types in the body including central nervous system cells. Pathology is caused by the virus infecting anterior horn cells in the spinal column.

Only motor cells, never sensory. Anterior horn cells of the spinal cord are affected, rarely the motor cortex and motor neurones of the medulla oblongata.
