How sweet of you to think of your wife. You didn't mention if you have children - but if you do - you could make plans to take the kids some where for the afternoon and let her have some quite time to herself to do with as she pleases. Maybe even schedule a massage or something for her.
Take your wife out after you have made all the arrangements for dinner & entertainment. Show her that she is still your special woman and how much you love her. Plan for a nice meal out and then maybe a play, movie, or some form of entertainment depending on her taste. Make it a real date so she feels very loved & special.

Go to the jewelry store and buy a beautiful ring, pendent, earrings etc. which ever you choose. Then make reservations at a nice restaurant, when your done have a nite cap at the bar before you head home, (have a friend, family member fix up your bedroom bathroom while you guys are out with candles, and rose petals) give her your gift, head home, walk her to your bedroom, take a beautiful bath together then have a great time everywhere else!!! Good luck and enjoy what ever you decide to do. I'm sure she loves you just the same!

Well, I don't know if you'd consider them creative, but I know I really appreciate it when my husband makes me a nice meal and sets the table and makes it pretty. Another thing he has done is prepared me a surprise bubble bath, set up music for me to listen to, lit candles, etc.

First, a hot bath with candles and a glass of wine. Second, a nice massage with some scented massage oils. Third, a dinner that you prepare and then clean up.

try cleaning up the house a little bit and cooking something that you know she loves. and if you have roses
try to put on on the table and spread some rose peddles all around the bed and some on the bed. maybe start up a warm bubble bath for her...
