No, but you have to apply for special privleges to get one registered. Every state is different, some easier than others. If the car was not sold here as an identical model (ie nissan skyline) then it is harder to get registered since it has not passed the federal crash testing and emissions. It is however relatively easy if the car was sold here (ie JDM Honda integra).

They're perfectly legal; I've seen a few licensed examples on the road myself. The lady who delivers mail in my neighborhood uses a RHD Saturn wagon.

No. You wouldn't want to regularly drive one. It's an adventure to pass another car.
