They do. I found quite a few by typing in "car seat cup holder" and clicking on the option to compare prices at Shopzilla. The first attachment below is a convertible car seat from Eddie Bauer that contains a cup holder attachment, so it appears that you could use the cup holder attachment even when the child reached the "toddler seat" stage.
You could always invent one with the modifications that make sense to you, though, if none of these are what you're looking for - I'm sure a lot of other mothers out there would appreciate it as well.

Some car seats have cup holders attached to them, cause the car seats that my kids have, do have cup holders attached to them. Plus it's depending on where you live and what mall you shop at. My 2 kids are 3 years and 161/2 months old.

There are car seats with cup holders. We bought an "Alpha Omega" car seat at wal-mart. It was about $130 but very nice. It not only has a cup holder, but it also has an adjustable head rest, arm rests and can recline. It has a 5 point harness and the straps on top are cushioned. Check It out!

Many car seats do come with cup holders. I have a Graco one for my son that has a detachable one and it REALLY comes in handy especially for long trips. It even has a little compartment which is good to put little bowls of his cheerios in for a fast snack on the road. I learned that just because it doesn't show the detachable holders on the floor models at stores, doesn't mean they don't come with them. If you look online at the different models it'll show you which ones come with cup holders and which ones don't. It's how I did my homework and knew what one was best.

We have an Eddie Bauer car seat 20-80 lbs, and it has a cup holder attachment on the side of it.

We have an Eddie Bauer Car Seat for our daughter and hers came with a detachable cup and snack holder. Maybe you just need to keep looking. We purchased this one at Babies R Us.

I have seen some with cup holders but mine is a year and a month and I am not about to give her a cup in the car yet. I don't like cleaning that much.

My older son's car seat has a cup holder on it. It snaps off for when you're not using it. It's not one of those convertible ones that's made from birth to 5 years old though--this is strictly made for toddlers. Maybe that's why?
