As far as the legality issue is concerned, it really depends on where you live. That said, it is not a safe practice. I did it in my younger years and luckily never crashed, but as I look back on that, the bike was a bear to control and I also worried about sudden component failure (wheels, frame, handlebars, etc.). The point that we are all making here is, DON"T DO IT unless it is a tandem.

No it is not illegal, provided both operator and passenger have a seat attached to bike. Bike need not be built for two. Passenger accomodations may be added to solo bike. For instance, adding a child seat (properly equipped) is legal.

Not on my tandem. It's not a good idea on a single. The extra weight changes the handling of the bike and makes it unsafe. Besides, one person has to do all the work and that's no fun.

Not illegal, may be fun but not a safe idea.

Teach the second person to ride the bike . . . it will be more fun for him / her.

yes it is because the back rim cannot support that much weight before failing and that rim can be expensive to replace ($ 230 and up). That's why it shouldn't be done.

Yes, you can get a ticket for it in most states. Common sense.

nowadays, probably...

when i was a kid, we used to "tote" another kid on our bikes all the time...he/she would either sit sidesaddle on the frame between the seat and the handlebars, or on the handlebars themselves...
