My name is... = Je m'appelle...
How old are you = 1. Quel âge avez-vous ? (that's polite french).
2. Quel âge as-tu? (if you know well the one you're talking to)
3. T'as quel âge? (if you're very familiar with the one you're talking to).
The choice is yours, depending on context. Context is very important, so be careful (to choose 2 or 3 in a formal situation is VERY impolite).
I'm 16 years old = J'ai seize(16) ans.
I like to play sports = cannot be plural in french, so you'll say instead "J'aime faire du sport".

The prononciation Hara indicated is correct, but her translation of the fourth sentence is maybe a little more complicated. "J'aime faire du sport" is ok as well.
