I think respect is earned. The reason for this being that in ninth grade I had a teacher who use to call all of us stupid and throw things at us. No one respected him. I walked out of class one day after being called stupid at least three times. He told me not to come back until I was ready to apologize. I said I would come back when he apologized. I had students coming up to me all day telling me he wanted to see me after school if I didn't mind. They all respected the fact that someone had finally stood up to him. When I went to see him he apologized. I guess he learnt a lesson in respect that day.

Both. I think you must both treat someone with respect thereby earning respect back. If you don't get it however, it's not your fault but theirs, well their choice I guess. It's a conscious choice to make after all. All you can do it lay out the carpet. If people refuse it, then they don't deserve yours either. That's my philosophy.
It doesn't always work of course. You have to TEACH children about respect and it's not something that just comes...especially when it's been absent for so long in someone....be it to give or to receive.

I'd say neither. True respect actually emanates from the very nature of someone sharing it. Once nurtured at the very onset of ones awareness, it could not be diminished by any experience or undesirable interactions with other people. However, the decision to convey it to someone (likable or otherwise) still lies on the one sharing it.

You don't earn respect. You demand it. Everyone is entitled to respect. If you don't demand it, people walk all over you. This earned crap started with E.F. Hutton a stock brokerage. Everybody knows how honest they are and how much respect they deserve. Oh, are they still in business today?
