Of course they know they do they just choose not to think about it. Masturbation is a completely natural thing whether the children are 15mo (like my son who always has his hand on his penis) or 15 years old or 50 years old.

Some of them know but don't want to admit it to themselves. Others think their child wouldn't do something like that. Whatever.
