He doesn't have hands. Unless you're referring to someone who is literally holding a pair of hands, like ones made of clay or stone, for example. Or if you're talking about hands of cards.
If you're talking about a person not having literal hands, it's the second.

Both could be correct, depending on the context.
He doesn't have hands means that he doesn't now have hands (they were cut off, for example he was a twice-caught thief in Saudi Arabia so they cut off both hands).
He doesn't have the hands would be correct in response to the question, Do you think he's make a good basketball player? No, he doesn't have the hands, meaning he's not good with his hands.

Both are correct depending on the meaning you want to give.
The 1st says that he doesn't have the hands as thou he should be in possession of a lot of them like the hands of the crash test dummies, for instance.
The 2nd one says that he's disabled, and that he has no hands of his own.
