My daughter had to go to an oral surgeon to be put out to have her teeth pulled. No dentist would put her out to pull them. It cost over $200 for 4 teeth with our insurance.

I would go for local anesthetic unless there's a severe, unusual problem. With the right dentist, there's no pain and they come right out.
