"What's up!" is just an informal greeting to acknowledge your presence. It doesn't require an answer. To be polite, you should acknowledge that acknowledgement by saying "Hi" or "Not much" or "Nothing new" or "Just the same" or anything like that or you can just smile.

What's up is a greeting asking you if there's anything new with you. or some new event. but usually, it's a colloquial term or slang for hello. I'm fine ,thank you . Or you could reply with nothing much and you

Basically they are asking you how you are doing and what have you been up to lately. You're reply says you're fine and haven't been doing anything exciting. And you're then asking them the same question in return.
