I am a clinical psychiatrist an I really don't quite understand your question. What I do understand is that you had "3 surgical" procedures performed either on one or both wrist for carpel tunnel. (This is water under the bridge). Confused about what does "rating" have to do with anything related to your surgery.

Doctor's will usually tell you that you have a 100% chance the surgery will resolve the problem or a 50% chance. When you are talking about 15%, 10% chance of surgery correcting the problem usually the doctor will not perform the surgery and the patient knowing how low the chances are they will refuse to sign for the surgery.

I assume that your profession requires repeated forceful movements with the wrist extened such as using a screwdriver, prolonged use of computer keyboard.

People who have diabetes or an underactive thyroid gland are at increase risk of developing carpel tunnel syndrome.

Hope that you doctor R/O diabetes or underactive thyroid gland. If not you really should have these two medical problems checked out.

You did not say if you started with corticosteroids injections into the nerve occasionally to bring temporary relief. The pain most have been severe or the muscle atrophies or weaken, and surgery was the best cost of TX.

Also I would assume that the doctor performed nerve conduction velocity test to be certain that the problem is carpel tunnel syndrome.

Hope that you are wearing a splint on both wrist for support.

Have you ever considered acupuncture as a natural course of TX. (Myself I would have opt for the acupuncture over the surgery).

Wish you the very best. And hope that you are living a quality life or will be soon.

Please excuse my english grammer. My primary language is Basque French.
