He could have a blader infection. Some times there is blood in the urine. That is nothing to mess around with, he needs to see a doctor immediately. And no, masterbating in your underwear will not give you an STD.

An STD can only be a possibility if he has had sexual contact with another person. If he has never had anything but his own hand down there, then it's not an STD. You do know that STD stands for sexually transmitted disease?
He may have kidney stones, an infection somewhere (bladder, urinary tract, urethra, etc...) or something else. He needs to go to a doctor. Only a professional can properly diagnose and suggest treatment.

Unlikely... sounds like a urinary tract / renal (kidney) infection... it could be chlamydia, gonorrhoea or even the early stages of syphilis.. has he had unprotected or any sexual contact in the last three months? Best advice.. see a specialist via referal through a GP.
If the pain is persistent and does not subside there are G/U clinics that you can attend without appointment.
Worst case scenario.. go to casualty.. best of luck... to your mate I mean..

Yes could be an STI/STD but he would only get that if he unprotected sex with an infected partner. It sounds more like he has done something to his urethra, bladder, prostate or testes. He should see a urologist as soon as he can.
