Yes,
Some convenience stores carry them, or check Costco or Sam's Club. They are 12oz small bottles but are made of glass!

Yes. either from foreign shops (mexico, asia, etc.) I lived in japan and that is what they mainly used. Coca-cola also makes a coffee beverage that comes in a bottle, called "Coca-Cola Blak" it tastes really good, but a little pricey for coke. =)

Yes, Coke still comes in glass bottles. I think if you go to a beverage store they might have them. Don't bother to look in a grocery store though.

Yes, here in México those are very popular, since they're cheaper than the plastic bottle and can versions.
And guess what? They can't be opened with a bare hand (twisting), you need a "destapador" (bottle-opener) (not like those used to open wine bottles).
And some people open them using:
*Keychains
*Cellphones
*Their teeth
*Knives
and many other things.

Sometimes I know where I live there are special deals in grocery stores when you can buy them. I do find that I like Coke more in the glass bottle than in the plastic bottle or in the can.

Yes they still have the small 6 or 7 ounces bottles that they make available from time to time in the US and on Latin and European countries they sold them regularly in all sizes.

Funny you asked, I was at Albertson ( I live in San Diego) and I saw them there. So I am assuming that most major markets will have it ..
