It depends on the grade level. I use the Glencoe grammar workbook with my ninth graders. I definitely recommend Glencoe over Prentice Hall--it's much more comprehensive and easier to read and understand. A good program online to practice grammar is MySkillsTutor; unfortunately, you have to pay to use the program.

the Harbrace Handbook. It's got everything in it, spelling, grammar, punctuation... it even tells you how to write bibliographies, works cited pages, and how to cite sources in a few writing styles. I know it has APA, MLA, and another one that I can't think of at the moment.

Can't go wrong with The Elements of Style. You can also read this book online.
