Presumably, they will have somebody help them fill in the form, and this is to record that the form was not filled out by the named person themself.
Or, it could be like the classic BIOS error, "No keyboard detected, press F1 to continue" - which is suprisingly NOT an urban legend, and you may even be able to see it yourself; if you have an Award BIOS, yank your keyboard and reboot.
