There are three options:
"Est-ce que tu veux être ma petite amie?"
"Veux-tu être ma petite amie?"
"Tu veux être ma petite amie?"
When you choose the last one, make it really sound like a question, your intonation has to go up at the end of the sentence, otherwise I don't think she's going to like it ; )

tu veux être ma petite amie? (pronunciation- too veuhs etreuh ma peteet amee?) it wouldn't normally have the s sound at the end of veux but as it is before a vowel you slur it

Marie has summed it up pretty well.
You can also ask "Veux-tu sortir avec moi?" or "Tu veux sortir avec moi?", which means "Do you want to go out with me". Ok, I've never quite understood if that meant "Do you want to be my girlfriend/boyfriend" in English, but in French, that's what it means.

If you're in France, go with either: "Veux-tu être ma petite amie?" or "Veux-tu être ma copine?".
If the girl is from Quebec, ask "Veux-tu être ma blonde?"
