There's only one way to get rid of any excess fat of any sort: cardio. You can't eliminate fat from one location on your body. You can definitely tone muscle, but the fat will still be there.

What I would recommend is cardio done for at least 30 minutes 5-6 times a week, and weight training at least 2 times a week. In those weight training sessions, focus on exercises that tone the triceps (muscle under your arms) and your pectorals (chest). You should do about 2 exercises per muscle group, where you do about 16 lifts per repetition (2-3 repetitions total.) The cardio will eliminate the excess fat and the weight training will tone those areas.

As for the condition you mentioned, I think your best bet is to discuss your options with your doctor. He or she may recommend exercise, hormone therapy, or in a worst case scenario, surgery.

The best advice that I can give you is get a personal trainer to give you some basic exercises to loose weight and to tone up. It is true that unless you are willing to make the area possibly larger, which i doubt you would want to do, then spot reduction is impossible. I generally recommend that for beginners they should go out and buy a intro Yoga DVD and this will usually work wonders. You feel great, be stronger, more flexible, better balance, and over all happy. if you combine that with eating 3 small meals a day along with 2 bananas in the morning (in between meals) and 2 small peaches in the afternoon (also in between meals) you'll do ok. As far as meals go, Always include a handful of either rice (white or brown with no salt or butter) or pasta (also with no salt or butter). Also you need veggies so include a handful of them too (either steamed, grilled, baked but also with out salt or butter added). I'm hoping your not vegan, but a handful sized piece of meat will also be part of your meals. Try to use meats which are lower in fat when possible, such as chicken breast and never eat hamburger. You can have steak twice a week but be cautious about what condiments are put on it. just so you know steak sauce is a no no, so is ranch or blue cheese dressing (fat free or not) also dont use more than a small spoonful of BBQ sauce. Most importantly drink water and only water from now on. (you will loose the want for sugar drinks in about 2 weeks of only water)

Special Note: a handful means a handful not a large steak.
another note: Brocolli and Coliflower are a must

Most importanly dont loose faith, your mental health is your most important factor to get you through this. don't quit! in as little as 2 weeks you'll see results.

Spot reducing fat is a myth. It's impossible. You can add muscle in an attempt to reshape. You must diet and get your bodyfat to a low level (10-15%).

Do chin pullups for the first problem. Pushups are good too. That will increase the muscle. To burn fat - do cardio. Have no idea about your second question.
