The first is similiar to parental sternness. Love for a person urges you to take stern measures for their own good, measures that may involve some discomfort for the recipient. Like letting a wild son sleep off his drunkenness in jail for a night, and refusing to rescue him. Tough Love committs interventions, takes away the car keys, reports the best friend for stealing fruit from a grocery store. Etc.
Sado-masochism is a mutually agreed -upon form of fetish sexual activity, usually involving physical pain of some sort (whips, chains, etc.).

sado-masochism is a pain/pleasure relationship that results in sexual pleasure.
"tough love" is being harsh enough to tell someone the truth and let them hit rock bottom until they snap out of it. It has more to do with awakening a persons awareness in themselves... it has nothing to do with sexual pleasure.

Your parents (hopefully) practiced tough love. They let you touch the stove once or twice to teach you about dangerous things. They let you date the trashy girl knowing full well she'd break your heart. That's like God. He has to let us fall down once in a while. It makes us who we are. It makes us stronger. It's relational.
Sado-masochism would be a totally differnt life for us. I doubt we'd ever have any form of happiness. I doubt there would ever be offers of forgiveness. There's no free will in that scenario.

tough love is a figure of speech, not an anything in particular, meaning you do something that upsets someone you love, but in reality it is good for them. S&M is kinky sex involoving pain. they have nothing to do with each other.
