I am just trying to figure out what the statute of limitations would be for a 6-year-old debt like this. I live in Illinois, the service was activated in Minnesota, but an actual contract was never signed. I know there are different time tables for various types of accounts.

The cell service does not fall under either written or open, regardless if you have a contract or not.

It falls under the UCC SOL - which is 4 years.

You answered your own question-- how can it be a written contract if you have no written contract?

Normally, you would have a written contract for the service, and then an 'open account' for the life of the service plan.
