usually it's body language.
confidence has a lot to do with it also, if a person is confident and is able (with body language) to let them know how they feel without being too blatant, then that usually qualifies as sensual/sexy.
i.e. for guys: take two women (twins, so there is no dispute on looks), they are very attractive and dressed very well (knee length grey skirt, slightly tight white blouse, hair in a bun, pumps... think sexy school teacher).
now one of these women is sitting up strait, legs crossed, drinking tea and has a book open in front of her. she is completely oblivious to the world.
the other is looking right at you. she is drinking a mocha with whip cream, and glances at you. a slow smile crosses her face when she, without breaking eye contact, starts licking the whip cream off of her drink.
now tell me, which one is more sensuous? which is more sexy? it's all about intent and confidence.
imagine a similar scenario with two guys (again twins), both dressed exactly the same. dark suits with a blue shirt open at the neck. your basic tall, dark, and handsome stereotype.
one of the guys is walking towards you and is lost in thought...
now imagine the other guy is walking towards you, a little smile playing accross his lips, his eyes never leaving yours...
which one is more sensual, more sexy? (sorry if my guy version is a little bare, just not good a visualizing a guy being anything else but something else in the background)
like i said, if everything about the person says, "i am interested in you" or "i want you". if they are confident enough to approach you and let you know who they are and what they think of you (not innuendo/crude remarks). that is usually sexy and sensual.
another way of thinking about it would be, think of what sexy and sensual mean... it all has to do with sex... so if someone is dressed sexy or sensual, they are usually covering up just enough to make you imagination perk up and take notice...

The body language, general attitude, and most of all their personality. The loudest most "out there" girl is usually not nearly as sensual or sexy as the seemingly innocent or shy on. This is just from my personal experience. Its how you present yourself that determines whether you're sexy or sensual, hope I helped.

A sexy confidence, A knowing stare. Slow , deliberate moves. Prolonged eye contact. caressing touch. A soft voice. expensive, suggestive clothes. An attitude that gently takes over and controls a situation. Surprised at nothing.

The way they look at you with their eyes and smile. You can tell if a guy is into you if he looks at you in a "love-struck" way . thats what makes then sexy and sensual.
